Hazing Reporting Form This form should be used to report acts of hazing that may violate Central Alabama Community College’s Student Code of Conduct and/or Alabama state law. Please provide detailed information regarding the incident you are reporting. If you believe a student is in immediate danger, please contact 911. Background Information Though not required, we ask that you please provide your name, student ID, and contact information. Your contact information will assist the College in case we have additional questions. Please report the acts of hazing even if you choose to omit your name and contact information.
